What to Track in Your RVU Log

Essential Information

CPT Codes

Record the 5-digit CPT code for each billable service performed. Be specific and accurate.

Date of Service

Always use the date the service was performed, not the date you're logging it.

Quantity

How many times you performed that specific code on that date.

Notes (Optional)

Brief context like "busy ER shift" or "2 complex procedures" - never include patient identifiers.

Hospitalists

Primary Care / Outpatient

Log after each clinic session. Record office visits by level (99202-99205, 99212-99215), preventive visits (99381-99397), and any procedures performed. Track both wRVUs and patient volume metrics.

Surgeons

Log operative cases as soon as dictation is complete. Include the primary surgical code plus any add-on procedures. Also log clinic visits and hospital consults. Monitor case mix and surgical volume monthly.

Emergency Medicine

Log after each shift. Group encounters by level (99281-99285) and track critical care, procedures, and observations separately. Calculate RVUs per hour to optimize efficiency across different EDs.

Common RVU Logging Mistakes to Avoid

Waiting Too Long to Log

Logging days or weeks later leads to inaccurate counts. Log same-day or at least within 24 hours.

Logging Wrong Date

Always use the date of service, not today's date if you're logging past encounters.

Inconsistent Logging

Logging sporadically makes data unreliable. Commit to daily logging or don't bother - partial data is worse than no data.

Including Patient Information

Never log names, MRNs, or identifiers. This violates HIPAA and is unnecessary for productivity tracking.
